    Geriatric Care (Old age care) / Continuing Care

    Geriatrics is a science of medicine which deals with medical & health problems of elderly people aged above 60 years. People who are old and frail or people with some chronic or degenerative diseases often need continuing support once they leave hospital, as their illness may prevent them from leading an independent life, Aastha can help them achieve best possible levels of ability and help to improve or maintain their quality of life.

    Geriatric care program is especially designed to support those patients who have long-term illnesses and which stay with them after they leave hospital.

    Long term Care

    Long-term care is broadly defined as an array of health care, personal care, and social services generally provided over a sustained period of time to persons with chronic conditions and with functional limitations.

    Functional limitations are typically assessed as limitations in activities of daily living (ADLs) or instrumental activities of daily living (IADLs). ADLs reflect an individual's capacity for self-care. They usually refer to the five basic functions: bathing, dressing, eating, transferring in and out of bed or chair, and using the toilet. Mobility assistance is often included. IADLs involve more complex tasks that enable an individual to live independently in the community. These include doing light housework, managing money, shopping for groceries or clothes, using the telephone, preparing meals, taking medications, and communicating verbally or in writing.

    We specialize in helping older people and their families with long-term care arrangements. We often undertake training programs in gerontology, social work, nursing or counselling. We also have extensive knowledge about the cost, quality and availability of services in an older person’s community. As a result, we can help:

    • Conduct care-planning assessments to identify problems and determine eligibility for assistance and the need for services;
    • Screen, arrange and monitor in-home help or other services;
    • Review financial, legal or medical issues and offer referrals to geriatric specialists to avoid future problems and conserve assets;
    • Provide crisis intervention;
    • Act as a liaison to families living away from the parent, making sure things are going well and alerting families to problems;
    • Help move an older person to or from a hospital, care home/old age home or nursing home;
    • Provide consumer education and advocacy; and,
    • Offer counselling and support.

    Terminal Care

    Terminal care is for those whose illness may no longer be curable. It enables them to achieve the best quality of life especially during the final stages of their illness. It may be provided at home, in hospital or in a hospice and it considers the person on the whole offering physical, emotional & spiritual care.

    The focus of Terminal care is to ensure the comfort of every patient during his final stages of life. This is achieved by relieving pain, managing symptoms such as anxiety, nausea, vomiting or shortness of breath, and by offering consistent emotional, social & spiritual support.

    Home Care

    Aastha's Home care service is designed with your need for personal care and safety in mind. We offer around the clock care by qualified caregivers. You and your loved ones will receive the highest quality of care and service that you expect. Home care aims to make it possible for people to remain at home rather than use residential, long-term, or institutional-based nursing care. We provide services in the client's own home. These services may include some combination of professional health care services and life assistance services.

    Professional Home Health services could include medical or psychological assessment, wound care, medication teaching, pain management, disease education and management, physical therapy, speech therapy, occupational therapy.

    Home Care is central to our service. We provide assistance with any of the following:

    • Help with all aspects of personal hygiene and dressing
    • Assistance with toileting and continence problems
    • Help with feeding
    • Teaching families basic nursing tasks
    • Nutritional advice
    • Administration of medicines
    • Helping with mobility
    • Care of pressure areas and pressure sores
    • Provision of emotional support
    • Providing someone to stay with the patient

    After performing an evaluation of the patient, we will design a customised care plan that matches you or your family member with a pre-screened caregiver who is compatible with your needs. All private duty caregivers work under the supervision of a registered nurse. Your caregiver will be an experienced, Hindi speaking, well groomed professional. They are available when you need them : daily, weekly, full-time, part-time or live-in.

    If your loved ones are faraway and there is a death in the family... we provide the facility to preserve the body, at the residence of patients or in the hospital premises, for upto 4 days.

    Aastha provides a unique facility for senior citizens, especially those who are very old and have difficulty in attending regular hospital OPD's, where the Doctors visits the patients at their residence at a time mutually agreed.

    We have a fleet of 4 ambulances, which includes Air-conditioned Cardiac & intensive care ambulances. We also undertake shifting of patients requiring life support systems to and fro from hospitals.
